Manuscript, paginated notebook bound with parchment with 'W Hey' written on the front cover. The fly-leaf is inscribed with 'Rd Hey 1781 William Hey junr. 1795'.

Contains 26 patient cases, some which were observations of patients treated by William Hey I and some cases where the patient was admitted to the Leeds General Infirmary. The notes are in three hands: the first two cases appear to have been written by William Hey I; the third and fourth cases potentially by Richard Hey; and the rest by his brother, William Hey II. The notebook is only approximately one third used, the rest of the pages are blank.
